Home health is the largest provider of long-term care. Nine million people received long-term care in 2014 and 1.3 million more people will need home-health services by 2025. Right now, 20 million people in the U.S. are 75-years-of-age or older and most want to age in place. In our session “Bringing Remote Care Services to Your Agency,” we discussed how our remote care services can help keep clients healthy and happy at home. Here’s a high-level overview of what these products can do for you:

What Is the value of Remote Care?
You will be able to keep nurses connected and informed, catch issues early to reduce hospitalization, and staff and patients will be happier with an easy-to-use system to support coordination of care.

How Will It Improve Your Home-Care Services?
A remote care program can decrease the likelihood of hospitalization by enabling patient-centered care. Clients can be monitored by their care team at all times so that problems can be addressed before they become a crisis.

Introducing Care at Home 2.0
Taking into account the success of Care at Home, and the feedback we received from customers, we have enhanced our technology to create the next version, Care at Home 2.0. To create this version, we approached it from a different perspective and incorporated design thinking. For the 2.0 version, we took a solution-based approach to solving problems through a collection of hands-on, problem-solving methods. We conducted field research with nurse ride-alongs, created medication reconciliation (med rec) prototypes, and approached scheduling workflows for a different angle. We also conducted a design thinking workshop to get to the root of the issue and find a creative solution.

After all that research, the newly revised Care at Home 2.0: A design-thinking approach, was created.

  • Enforces Medicare work-week compliance
  • Enables clinicians to manage their schedules
  • Provides at-a-glance patient and case summary
  • Allows for updates to patient medications and vitals, outside of an assessment
